The program has already resulted in both improving care quality and lowering costs by avoiding unnecessary hospitalizations and emergency room visits.
To receive recognition from The National Committee for Quality Assurance (NCQA), St. Martin Parish Community Health Center demonstrated the ability to meet the program’s key elements.
